To prepare for your anaesthetic, please follow these steps:

1. Download the pre-anaesthetic form by clicking this button: ​
  • As soon as you are given the date of your surgery, please complete a pre-anaesthetic assessment form to avoid any delay.
2. Complete the form and keep a copy of the completed form until after your procedure.
3. Submit the completed forms using either the button at the end of the form, or by attaching it to     an              email addressed to drwlim@gmail.com
  • If we require further information after your form has been submitted, we will contact you via phone or email
4. The nurses from St John of God hospital will usually try and contact you the day before surgery     regarding admission and fasting times.
  • If you do not receive a telephone call by 3 pm, please contact the hospital reception on (08) 9722 1600 to confirm your admission and fasting details.
5. You may need to have a pre-anaesthetic clinic appointment if:
 
  • You are having joint replacements, gastric surgery or complicated major surgery
  • You have a complicated medical or anaesthetic history
  • Your surgeon or GP requests a pre-anaesthetic assessment

Ideally, this assessment should occur at least two weeks prior to your surgery.
